Clearly they never fixed the problems since it just exploded, killed 15 people, injured hundreds and destroyed most of the town.
Cited and fined, for one of the biggest most damaging, events caused by willful negligence in the history of the USA!
In woefully unregulated Texas, that paragon of self reliance and self responsibility.
